Renew Your Health Card
You need to bring your Ontario Health Insurance Plan (OHIP) card each time you come to MedCare Clinics to receive services. It must be a valid health card for our clinic to collect payment for your services from the Ministry of Health.
If your health card has expired or the number is invalid, please renew your health card online. Once renewed, return to this page and update your health card information by completing the form below.
Provide Your Updated Health Card Number to Our Clinic
To ensure accurate billing covered under the Ontario Health Insurance Plan (OHIP), we require your current OHIP card number along with the version code. Without a valid version code, access to medical care may be impacted.
